If the communication chain is broken or has deficiencies, this can lead to misunderstandings and errors which can affect the entire team and especially the patient. In the healthcare setting, different methods of communication are used, such as written notes and handoffs, telephone/fax, note boards, emails, verbal and body language. However, the most critical information is still written by hand, via email or text message, which can result in heavy penalties in the event of an error or miscommunication. It is essential for interprofessionals to communicate with the health care members with whom it is necessary to communicate to achieve the best care and outcomes for the patient and as health care.
