Many of these tasks are quite mundane but well documented, but still present the same dangers to those involved in the maintenance process. If a task becomes so ritualistic in nature, we forget to practice all aspects of the process at hand, even when it is documented procedurally. Due to an individual's complacency, a document defining the process is opened quickly, briefly examined and immediately becomes a hindrance to the process. By transforming a process into a simple reflex act and extending the time between these reflexes, we only relax the procedural techniques. Supplement the mundane routine by adding a variable, like changing a toxic gas control valve, and there's a good chance someone will end up in the emergency room or worse. This is why the term “specific tasks” was emphasized previously. Often the specific task turns into a large task. At this point, the PTP form should be used, but it is often overlooked and/or downplayed in importance by the customer and technician. Again, the combination of stressors, loss of time and income, and complacency often leads to something happening.
