He seems alert and oriented x3 and happy. Bowel sounds present x 4. Pt. has a red area on his coccyx. Silvadene treatments have been started. Pt. Has a fungal lung infection with a suction pleural drainage tube inserted into his chest. Pt is extremely thin with poor skin turgor with a diagnosis of cachexia (wasting) secondary to malnutrition and infection. Psychology Dr.W. There are physiotherapy, occupational therapy and infectious disease consultations if necessary. He lives with his wife to whom he has been married for 56 years. He does not use a hearing aid but he suffers from hearing loss.Pt. is able to perform all ADLs with limited assistance. He wants to get better and leave the HSP. Pt. He said “90 days is too long to be here.” Pt. states that he is concerned about maintaining his tube site when he gets home and doesn't think his wife can do it for him.
